SCHOOL COMMUNITY OUTREACH EXPERIENCE
Learner Profile:
SCORE is a program for 14-15 year old students who have struggled with attendance and credit accumulation for at least one semester in a Halton District School Board secondary school.
Description of Program:
The program operates in the four communities of Halton through Gary Allan High School. The program provides for individualized instruction in compulsory courses, credit recovery opportunities, and co-op or work experience opportunities. Students must be full-time in SCORE initially but can later return for part of their program to do regular course work at their home school.
Exit Pathway:
Students typically continue their studies at their home school after leaving SCORE or may consider other Gray Allan High School programs like STEP or Ex-cel.
Application Process:
Intake and review occurs on a monthly basis from October through April at each site. At that time, the intake case is presented by the home school administrator or designate to the intake team. The student and parent/guardian must attend the intake. The following steps outline the application process:
·A variety of school-based interventions have been implemented and the
student/parent are interested in pursuing the SCORE referral.
·The student is referred to SCORE by the School Resource Team at the home
school.
·The school social worker or administrator contacts the SCORE teacher in their
community to initiate the application.
·The school presents the student at a SCORE intake meeting.
·The intake team reviews all cases presented and makes decisions.
·Decisions are communicated to all schools within 48 hours by the teacher.
