If your child already attends a Halton District School Board School then...
Just call us at (905)844-9322. We will arrange to have your child’s records transferred to Maple Grove Public School electronically.
If you have moved or are registering your child for the first time then...
Ensure that you live within the Maple Grove Public School boundaries by accessing School Locator on the Halton District School Board website at www.hdsb.ca or by calling the Planning Department/ Boundary inquiries at (905) 335-3663, ext. 3240.
If you have a child ready to start Kindergarten, registrations for the following school year, begin after the Christmas holidays.
Your child must turn 4 by December 31st to be registered for Junior Kindergarten. Children who turn 5 by December 31st will be registered for Senior Kindergarten. All Kindergarten classes at Maple Grove are blended JK-SK classes.
Documents needed to register.
You will need to provide ALL of the following information to register your child for school: ***Please note: Only original documents will be accepted.***
1) Proof of child’s age - original birth certificate or passport
2 ) Proof of address - copy of offer to purchase or lease agreement or property tax bill
3) Your child’s health card
4) Proof of child’s citizenship - To register your child must be a Canadian Citizen or Landed Immigrant. If your child is not a Canadian citizen or Landed Immigrant, you will need to contact the Immigration and Visa Co-ordinator at the J.W. Singleton Education Centre at (905) 335-3663 ext. 3307.
5) Birth certificate, passport or Record of Landing (IMM1000).
6) Proof of custody - if you are not the child’s parent and the child is under 18, you must provide proof of custody.
7) Immunization record - proof that your child has been immunized, according to the recommended immunization schedule in Ontario, against the Diphtheria, Mumps, Polio, Red Measles, Rubella and Tetanus.

· To verify that your child’s record is up-to-date
· To obtain the exemption form if your child has not been immunized for medical reasons or because of a conflict of religion or conscience
· For information on how to have your child immunized if you do not have an Ontario health card and can’t afford a doctor’s fees for immunization.
