P R O G R A M   D E S C R I P T I O N	Transitions Elementary Day Treatment Program   This program focuses on students who are experiencing learning difficulties, mental health issues, emotional and behavioural problems. Students receive support through daily group therapeutic activities, as well as individual sessions weekly. The academic program focuses intensely on the delivery of both language and math.The program covers many areas of the curriculum including: physical education, health, science, with a concentration on mathematics and literacy. The classroom provides a small learning and therapeutic environment. The classroom functions as a team-based experience, where the children can rely on support both academic and therapeutically throughout the day. Length of stay is determined on a per child basis and will be discussed throughout the program. Students may be referred to the program by their home school or by parent/ guardian. Referrals should be directed to the Program Supervisor.
